  Health services provided include one-on-one counseling, clinics and screenings, education regarding community member and caregiver knowlege of diabetes management, assisting in the development of effective strategies to address diabetes and other chronic illnesses and monthly wellness and diabetes support group meetings.
• Home and Community Care - Assessment, Monitoring, Nursing

          The Home Care Nurse supervises six Home Support supervises 
          six Home Support Worders and two Homemaker/Housekeepers.
          Ongoing assessment and monitoring of clients and individual
          nursing service is provided as needed.
