This program is designed for individuals of any age who have an assessed need, are currently living on reserve, and want to maintain their independence and health at home. Assessments are completed by a nurse. Home supports are then set in place based on the results of the assessment. Supports vary and may include advocacy during hospital stays or medical appointments, light meal preparation, assistance with activities of daily living, medical supplies, and medical transportation. Anyone can refer clients to the program; however, the client must consent to the confidential initial assessment. Based on the assessment, a Homemaker or Home Support worker will be assigned to visit your home and a nurse will work with the client and/or family to plan care around the identified needs.
