Call the Health Department at Community Services if you have a medical concern or require assistance and would like to arrange for an assessment. This program is designed for individuals of any age who have an assessed need, are currently living on reserve and want to maintain their independence and health at home. Assessments are completed by a nurse and based on the assessment, home supports are set in place. Supports vary from client to client and can include advocacy during hospital stays or during medical appointments, light meal preparation, assistance with activities of daily living, medical supplies, specialized medical equipment and medical transportation. Anyone can refer clients to the program, however, the client must consent to the confidential initial assessment. Based on the assessment, a nurse will work with the client and/or family to plan care around the identified needs and either a Homemaker or Home Support worker will be assigned to visit your home.
